A non-liability clause is printed on the back of every claim check stub and a work statement form is also included on back of the office stub. There's a suitable space on claim check for rubber stamping name and address. 

THE NON-LIABILITY CLAUSE IS PRINTED ON THE REVERSE SIDE OF THE CLAIM CHECK STUB. It reads:
